	Please download, print and fill out all of the forms below. Send to DBC: (60 Quentin Rd. Lake Zurich, IL 60047) Enrollment Application Character and Conduct Emergency Form Athletic Waiver Form Credit and Check Form Send to your previous school(s): Request for Official Transcript form Please download and print copies of the "Request for Official Transcript". Mail a request to every institution listed above. Transcripts must be sent directly to DBC Undergraduate/Graduate Admissions office. Send to the people you have asked to submit the reference for you: Reference Questionnaire Please download and print out three copies of the "Reference Questionnaire" form. Complete the applicant portion of each form, include your signature and the date, and give one to each person you have asked to submit a reference for you. Please give the full name, address, and position of each individual.
